Building a Community Collaborative: Evidence-Based Interventions that Bring Together Healthcare Providers, Community-Based Organizations, and the Criminal Justice System

Wednesday, April 12, 2017, 1 to 2 p.m. EST — Individuals with complex challenges arising from chronic health conditions, mental health and/or substance-abuse disorders, or involvement in the criminal justice system are among the highest-cost utilizers of the healthcare system. A multi-pronged Community Collaborative can ensure evidence-based interventions that identify and effectively treat high utilizers – helping to keep them out of the emergency room and out of jail.
